Upper Perk Kiwanis Baseball Emergency Release

I/we certify that player has adequate medical and hospitalization coverage to participate in Upper Perkiomen Kiwanis Baseball and that I/we will accept full responsibility for any expenses incurred as a result of accidental bodily injury that may occur while participating in practices or games under the supervision of the Upper Perkiomen Kiwanis Baseball personnel. In the event of an emergency, I give permission for my child to be treated by a community doctor, emergency room, or hospital and to be transported by private car or by an emergency vehicle, if necessary.

Insurance Waiver / Hold Harmless Clause:

The Upper Perkiomen Kiwanis Baseball Program is made up solely of volunteers with the purpose of helping the players of the Upper Perkiomen Valley learn about baseball and learn proper skills of the game. By registering, the parent/guardian of the participant(s) agree to hold harmless all volunteers of the Upper Perkiomen Kiwanis Baseball Program, all members of the Upper Perkiomen School Board, The Perkiomen School, all coaches and administrators for the loss of any material, equipment, and injury which may occur while using the facilities of the Upper Perkiomen School District or The Perkiomen School, while participating in the Upper Perkiomen Kiwanis Baseball Program. In addition, the parent/guardian and participant agree to hold harmless all parties identified, above, for all acts of negligence.
